Before Your First Visit
Please be sure to create your online profile and complete new patient paperwork 24-48 hours prior to your first visit . You will received an invite to our new online platform via janeapp.com, with instructions how to access the intake forms. If you have any prior imaging ( x-rays, MRI's, CT scans), post surgical reports, or prior records related to your care, please have them with you. Should we need to request records, we will inform you at the time of your visit.
Arrival
Please arrive 5 -10 minutes prior to your scheduled appointment. If your running late please inform us as soon as you can. Patients more than 10 minutes late will need to reschedule, depending of the providers schedule. Feel free to bring your own athletic clothes or use our shorts, t-shirts, gowns, and socks. We have changing rooms with secure lockers for your convenience.
Consultation
The initial consult with your provider is designed to review your history, understand your current concern(s), and understand your health goals.
Examination
Your provider will perform a complete exam which may include range of motion, muscle strength, and reflexes. Other tests may include neurological, orthopedic, and postural evaluations. It is very important to us that you are comfortable with each step of the process. Communicating your questions and/or concerns with your provider is welcome every step of the process.
Treatment
At the conclusion of the exam we will begin a short treatment, which may include hands on mobilization, modalities, therapeutic exercises, massage therapy and/or recommendations. Treatment is interactive, so you can express concerns and preferences regarding styles of treatment at anytime.
Care Plan
At the end of your first visit, the provider will go over their findings and answer questions for you.
Our care plans will provide the best treatment and wellness program for your needs. If a referral is needed for X-rays, MRI, or another medical specialty, our doctor will make that referral.
